Privacy notice

This notice describes how Axis Smile Studio, PLLC ("Axis", "we") collects, uses and protects your information, including protected health information (PHI), consistent with the Health Insurance Portability and Accountability Act (HIPAA) and applicable Texas law. It is a demonstration document for a fictional practice.

Information we collect

Contact details you provide (name, email, phone), enquiry content, and — for patients — clinical records including scans, images and treatment history.

How we use it

To respond to enquiries, provide and coordinate dental care, arrange financing you request, meet legal and regulatory obligations, and (only with your consent) send you updates. We do not sell your information.

Your rights

You may request access to your records, ask us to correct them, request restrictions on certain uses, and receive an accounting of disclosures, as provided by HIPAA. You may file a complaint with us or with the U.S. Department of Health and Human Services without retaliation.

Security & retention

We apply administrative, physical and technical safeguards to protect your information and retain records for the periods required by Texas dental record-keeping rules.
